By signing and initially each term in this guarantee, the puppy buyer certifies that he or
she has read, understood and agreed to the terms and conditions in the contract and
legally obligates himself or herself to follow the terms in the contract. The breeder, by
signing the contract, agrees to honor the terms of the guarantee, if needed. If you are
printing this form to send through the mail, please copy and paste the text into a word

Puppy Guarantee & contract
_____ Deposit of $_________ is due upon acceptance of the signed contract and
puppy buyer info sheet by breeder to reserve the puppy of your choice and is NOT
REFUNDABLE FOR ANY REASON except if a hereditary/genetic condition arises in
reserved puppy before puppy is purchased.
_____ The balance remaining at time puppy is ready to go home is due IN CASH at
time the buyer picks up the puppy. AMOUNT DUE AFTER DEPOSIT: ____________
_____ Puppy comes with 1-year genetic/hereditary defect guarantee covering all
life-threatening conditions. Any condition discovered after taking puppy home must be
documented by a licensed vet and paperwork, including all tests ran, must be available
for breeder's vet to review before guarantee will be honored.
_____ Puppy must be taken to buyer's vet for a wellness check-up within one week
of taking it home to validate the guarantee. If anything is found that was not
documented at breeder's vet, buyer will immediately contact the breeder.
_____ If puppy is to be returned on the guarantee, it must be done so at the buyer's
expense.
_____ If a puppy is returned on the guarantee after the first week, a puppy from a future
litter will be substituted when it is available. (Because I can't guarantee the sex of
puppies born before-hand, you may need to wait more than one future litter for the sex
you prefer.) NO CASH REFUNDS WILL BE MADE.
_____ After taking puppy home, all vet bills are the responsibility of the buyer while the
puppy is in the buyer's care, even if puppy is returned for any reason.
_____ Puppy has been health checked by breeder's licensed vet, Gerald Uzarski or
Greenville, PA, appropriate vaccinations given and wormed several times, including a
comprehensive physical examination and only puppies passing a health examination
and deemed healthy and fit with no known hereditary or genetic conditions are placed
with buyers.
_____ The breeder is not responsible for replacing puppies returned due to buyer
negligence and the guarantee does not cover environmental illness, injury, accidental
poisoning, choking, car accidents or lack of proper vet care, feeding, bathing, housing,
etc. but will take any puppy back at any time rather than it being given away or put in an
animal shelter.
_____ Puppy has had dewclaws removed.
_____ Each puppy comes with a puppy pack, including food sample, health papers
from the vet, AKC registration papers.
_____ The puppy will be registered with AKC at the buyer's expense, if the buyer
chooses to do so.
_____ Buyer agrees he or she will not resell or give the puppy/dog away without
contacting the breeder beforehand.
_____ Puppy is not available for pickup until after 8 weeks of age. Or when puppy
weighs enough. (Because of the small size of puppies and how fragile they are when
they are babies, please don't ask to take your puppy early!)
